Here are the points which are examined by the Google for detecting the quality content -

1. Unique contents
2. Length of content
3. Keyword
4. Infographics in the content
5. Readability
6. Presence of adds in the praportion

Google will be able to detect that the text you entered correlates to the form, you won't be penalized for duplicate content. Google will only hold content against you when the entire (or majority of) page content is duplicated (often including meta tags etc),

There are certain things that Google look at when they're judging your website's quality. It's important that your website isn't doing things that Google ask you to avoid. Some of them are:

1. Avoid hidden links or texts on your website
2. Avoid tricky redirects on your website
3. Avoid creating pages with no or little original content
4. Avoid link schemes
5. Avoid creating automatically generated content
6. Avoid filling pages with irrelevant keywords
