Which Page is called the perfect optimized page? If anyone tell you to optimize a page perfectly then how could you do? Which factors you should remember to make it perfect? Which on-page task you should implement to make your page perfect?

Unique content, Title meta optimization as naturally, no stuffing, internal linking, good PR and targeted keywords.
If these all are covered on any site or web page, it's called perfect optimized web site.

There are a couple of factors when it comes to optimizing a page. First you want to have good content. It should be over 500 words. Then you want to target a certain keyword on the page. Put the keyword in the title and url. Try to use that key word about once every couple of sentences. Then you want to bold the keyword. Try to use the keyword in the header tag. If you have images use the keyword in the alt tags.
