1) Splash pages:

5) Unfriendly URLs:

Most blog or CMS platforms have a friendly URL feature built-in, however, not every blogger is taking advantage of this. Friendly URL’s are good for both your human audience and the search engines. The URL is also an important spot where your keywords should appear.

Beside these, I have seen people not giving importance to the users perspective while designing the site. It should be simple with smooth navigation.
And yes, Flash should be a big No.

Yes I am not totally agree with your first four points normally we can insert and use these feature by following Google Guild lines. I know that Google tell to build an high speed site but its not meant that you should not use high technique at site. There are other option to use flash like swf object file that make your flash readable to search engine. As we are making our site to our visitors not google is about to pay

Yes last option is quite right SEO must think about right URL its really big issue and big factor in search engine ranking
